meeting of shareholders at 7:00 p.m. Mountain time on Tuesday, June 23, 2009, at
the Hampton Inn & Suites located at 851 West 1250 South, Orem, UT 84058. The
Board of Directors has fixed the close of business on June 1, 2009 as the record
date for determination of shareholders entitled to receive notice of and to vote
