Health2 Resources Announces News Portal, E-News Brief Focused on Patient
Centered Medical Home

One-stop news source keeps readers informed about key delivery model for
health reform

WASHINGTON, Sept. 4 /PRNewswire-USNewswire/ -- Health2 Resources announced
today free access to its Web portal, H2RMinutes, a one-stop source for news
about the patient centered medical home (PCMH). H2RMinutes
(http://www.h2rminutes.com) is designed specifically to keep health care
policymakers, providers, employers and suppliers informed about the impact
that the PCMH model is having on the national health care reform debate. The
PCMH model is a fundamentally different approach to the way health care is
delivered, putting the patient at the center of continuous, uninterrupted care
that is managed and coordinated by a primary care provider. As Congress
debates proposals to reform health care, the PCMH has emerged as a means to
improve care delivery and lower costs.

The H2RMinutes Web portal supports a weekly free e-news brief, currently
delivered to more than 18,000 national, regional and state PCMH stakeholders.
Readers look to H2RMinutes to obtain a summary of timely and up-to-date news
about community impact and national dialogue relating to the PCMH. H2RMinutes
enjoys educational sponsorship from the Patient-Centered Primary Care
Collaborative (PCPCC), a coalition of major employers, consumer groups,
patient quality organizations, health plans, labor unions, hospitals,
physicians and many others who have joined together to develop and advance the
PCMH.

"H2RMinutes delivers the breaking news and timely research that stakeholders
need to stay in the know about the PCMH," said Katherine H. Capps, president
of Health2 Resources. "Readers can scan or delve more deeply into the
articles and research linked to the H2RMinutes portal. The articles are
relevant, and help them stay informed of meaningful market examples about how
PCMH is improving health care delivery, how providers are adopting the PCMH
and using health information technology, and what policymakers are doing to
extend the reach of the medical home."

A Health2 Resources editorial team developed and implemented the H2RMinutes
portal as an internal initiative, in recognition of the need for a centralized
source of breaking news and new research on the PCMH. The Web portal and news
brief's audience includes providers, suppliers, consumer advocates, policy
makers, and primary care professionals, interested in the PCMH.

The Web portal is a one-stop source for archived news and information about
the PCMH. Linkage is made to news, current reports, membership information
and educational materials from the PCPCC.
