Third Annual PCPCC National Summit: All Eyes on The Patient Centered Medical
Home

Convening in Washington, DC, October 22, 2009


WASHINGTON, Oct. 16 /PRNewswire-USNewswire/ -- The following is being released
by The Patient-Centered Primary Care Collaborative:


WHO: The Patient-Centered Primary Care Collaborative, a coalition of major
employers, consumer groups, organizations representing primary care providers,
and other stakeholders who have joined to advance the patient centered medical
home.


WHAT: The Third Annual PCPCC Summit. More than 400 CEOs, senior executives and
government officials unite around the Summit's goal to build, expand and
support the PCMH model. Particular focus will be aimed at the role of the PCMH
in the current health reform legislation and on results from demonstrations in
the field.


WHEN: Thursday, Oct. 22. Registration starts at 8 a.m., sessions run from 9 to
4:30 p.m. Eastern

The shape of health care is changing, and the patient centered medical home is

introduced at the Summit. Across the nation, dozens of patient centered
medical homes are already organizing or operating at the local, regional and
state levels. Proof in Practice: A compilation of patient centered medical
home pilot and demonstration projects reports the progress of pilots and
initiatives, providing descriptions and details of how they are structured and
what they are achieving


Consumer engagement and collaboration in care are vital components for PCMH

the medical home presents resources for consumer engagement developed by the
PCPCC and partner organizations, as well as industry resources and the latest
