The Dublin, Ohio-based company said it has received two reports of deaths and one report of injury related to the Alaris Pump and that deaths had not been confirmed as definitively caused by the issue.

Cardinal Health said the recalled Alaris Pump units, which were model 8100 and shipped prior to Sept. 27, may contain occluder springs that are bent, broken, nested or missing. Such misassembled springs could lead to overinfusion that could result in serious adverse health consequences or death.

The company has a 24-hour information line for questions about the recalled pump at 1-800-625-6627. (Reporting by Lisa Baertlein; editing by Carol Bishopric)
