D'Arbeloff has served as a Director of Biolase Technology, Inc. since 1996, as lead independent director from March 2006 through May 2006, and as Chairman of Board since May 2006. Since February 2008 Mr. d’Arbeloff has served as a Managing Member of Disruptive Capital Partners, LLC, the managing entity of DCP Premiere Fund, a hedge fund investing in long/short positions in U.S.-based equities. Since 2003, Mr. d’Arbeloff has served as Managing Member of Opus Venture Group, LLC, a company dedicated to providing innovative products for various retail outlet channels. Since 2000, Mr. d’Arbeloff has served and continues to serve as Chairman of the Board of Big Idea Group, Inc., a company that links inventors with companies outsourcing innovation. From 1996 to 2000, Mr. d’Arbeloff served as Chief Executive Officer of Retail Solutions, Inc., a small early-stage private company. Prior to joining the company as Chief Financial Officer in April 2008, Mr. Mulder was the Chief Financial Officer and Chief Operating Officer of American LaFrance, a private equity turn-around company focused on manufacturing emergency vehicles, from January 2006 until August 2007. In January 2008, American LaFrance sought protection under Chapter 11 of the U.S. Bankruptcy Code, and emerged from bankruptcy four months later with approximately 90% of its creditors supporting its plan of reorganization. Most recently he was CFO at Southern California-based North American Scientific, a medical device company that manufactures, markets and sells products used primarily in the treatment of prostate and breast cancer. The prostate business was sold to BEST Medical in April 2009 and the breast business was sold to Portola Medical Inc in May 2009. Prior to North American Scientific, Scott was CFO for Irvine, CA-based Alsius Corporation from January 2006 to August 2008. At Alsius, Scott managed the initial IPO process and was a key member of the negotiating team during the resulting merger with Ithaka Acquisition Company, a specified purpose acquisition company. From September 2001 to March 2005, Scott was CFO at Irvine-based Irvine Biomedical, which was acquired by St. Jude Medical, Inc. in 2004 and designs, develops, manufactures and markets electrophysiology products for the minimally-invasive diagnosis and treatment of cardiac arrhythmias. From July 2001 to December 2002 he was CFO and a member of the Board of Directors of Irvine, CA-based Pain Concepts Inc. that sold its product line to Stryker Medical Inc. in October 2003. | Anderton, Robert | Robert M. Anderton, DDS, has served as a director of Biolase Technology, Inc. since May 2004. From 1999 to 2001, Dr. Anderton served as the President of the American Dental Association (ADA) as well as holding many official roles with the ADA, including Trustee, Liaison to the Commissions on Dental Accreditation, Council on Education, Government and Legislative Affairs. Dr. Durrie has practiced ophthalmology in the Kansas City area since 1990, establishing Durrie Vision, PA in October 2002. Mr. Waller has extensive experience in the medical device field that began at Irvine, CA-based American Hospital Supply Corporation (AHSC). Waller spent more than a decade at AHSC, where he served as Harleco Division Operations Manager, McGaw Laboratories Division Plant Controller, and Ormco Division Vice President and Controller, until the sale of AHSC to Sybron Dental in 1982. From 1980 to 1989, he was Vice President and Controller at Ormco and, from 1989 to 1993, he was Vice President, European Operations at Kerr Corporation, each wholly owned subsidiaries of Sybron Dental. From 1993 to 2005, Waller served as Vice President Finance, Chief Financial Officer, and Treasurer at Sybron Dental, where he managed the financial and information technology integration of Kerr and Ormco into Sybron Dental, developed the strategy for consolidating and integrating some 30 strategic acquisitions, managed the spin off of Sybron from Apogent Technologies, and led Sybron's corporate finance activities, including a secondary public stock offering, a new financing facility, and the refinance of the company's existing debt.
