Name Description

Izzat bin Othman

Mr. Izzat bin Othman serves as President Commissioner, Independent Commissioner of PT Millennium Pharmacon International Tbk since July 9, 2015. He was appointed as Independent Commissioner on May 26, 2011. He serves as Partner of law firm of Messrs. Azzat & Izzat. He graduated with an LLB (Hons) in April 1985 from Universiti Malaya. Subsequently, he was admitted as an Advocate and Solicitor on January 25, 1986. He was formerly a Director in AFFIN Securities Sdn Bhd and BH Insurance Berhad and has experience relating to litigation, conveyancing and corporate matters. He also sits as Chairman and committee member for the Disciplinary Committee of the Malaysian Bar, as well as Board of Directors for several private companies.

Mohamad Muhazni bin Mukhtar

Mr. Mohamad Muhazni bin Mukhtar serves as President Director of PT Millennium Pharmacon International Tbk since October 18, 2012. He obtained American Associate degree from Indiana University in 1988, Bachelor of Business Administration in Management Science from University of Iowa in 1990, Master of Business Administration from Ohio University/Universiti Teknologi MARA in 1993, and Master of Science in Business Leadership from Newcastle Business School, Northumbria University in 2010. He has worked with London & Pacific Insurance Bhd in 1991, Plus Expressway Bhd from 1993 until 1996, Touch and Go Sdn Bhd, Faber Group Bhd and Faber Facilities Sdn Bhd. He is a member of Company's Management Risk Committee.

Ahmad bin Abu Bakar

Mr. Ahmad bin Abu Bakar serves as Director of PT Millennium Pharmacon International Tbk since May 23, 2012. He is a registered pharmacist in Malaysia. He obtained Bachelor of Pharmacy from Bradford University, UK (1993). He has over 20 years of pharmaceutical experiences and has worked in various fields of pharmacy, including retail and wholesale pharmacy, manufacturing and private hospital. He has worked with Pharmaniaga Logistics Sdn Bhd (Northern Branch, Penang). He is a member of Company's Management Risk Committee.

Ivan Hanafi

Mr. Ivan Hanafi serves as Independent Director of PT Millennium Pharmacon International Tbk since April 6, 2018. He obtained Bachelor of Science in Electronic Education from Universitas Negeri Jakarta (1986), studied Master of Education at Universitas Yogyakarta (1996), Bachelor of Engineering in Electronics Engineering from Universitas Indonesia (2000), and PhD in Technical and Vocational Education from Universiti Putra Malaysia (2007). He was Training Coordinator for Technical Training Center, Siemens Indonesia Company (1994-1995), Consultant to The Ministry of National Education, assisted the Terro-Technologist International Consultant to review preventive maintenance and school-based management programs used in Junior and Senior Secondary Education (1997 – 2002), Chairman of the Committee for doctoral scholarship and short-term training abroad on BERMUTU program, Directorate General of Higher Education, Ministry of Education and Culture (2008-2010), Coordinator of the South East Asian Vocational and Technical Education Research and Networking (SEAVERN) Project for Indonesia (2009-2010), Head of Center for Educational Policy Studies, Universitas Negeri Jakarta (2011-2014), and Chairperson of Master Program for Technical and Vocational Education, Universitas Negeri Jakarta (2013-2017).

Mohamed Iqbal bin Abdul Rahman

Mr. Mohamed Iqbal bin Abdul Rahman serves as a Commissioner of PT Millennium Pharmacon International Tbk since May 23, 2012. He obtained Bachelor's degree in Computer Science from Universiti Putra Malaysia. He joined Pharmaniaga in April 2011 as Information Technology Director and was appointed as Chief Operating Officer on June 1, 2012. He has over 25 years of experience, mainly in the field of System Improvement and Operations Management. He is a member of Company's Nomination and Remuneration, and Management Risk Committees.

Noraini binti Mohamed Ali

Ms. Noraini binti Mohamed Ali serves as a Commissioner of PT Millennium Pharmacon International Tbk since May 23, 2012. She holds Bachelor degree in Finance and Accounting from Liverpool John Moores University, UK, and is a qualified accountant of the Association of Chartered Certified Accountants (ACCA). She was a member of Company's Audit Committee. Currently, she is member of Nomination and Remuneration, and Management Risk Committees. She was the Senior General Manager at Pharmaniaga Group Corporate Finance and was subsequently appointed as Procurement Director in April 2011.

Nyoman Kumara Rai

Dr. Nyoman Kumara Rai serves as Independent Commissioner of PT Millennium Pharmacon International Tbk since October 21, 2014. He graduated with distinction from Faculty of Medicine, University of Indonesia (Universitas Indonesia) in 1968. In 1971 he finished his post-graduated study at London School of Hygiene and Tropical Medicine in London. in 1976 he obtained his Master of Public Health in health planning with emphasis on socio economic development in school of Public Health, University of Michigan, USA. He started his career in Ministry of Health from 1968-1988 as staff of Directorate General-Control of Communicable Diseases, starting with Smallpox Eradication Programme, Chief Sub-directorate of Immunization, Chief Planning Unit, Chief Sub-directorate of Malaria Control and Director Diseases with Direct Transmission and finally Secretary Directorate General-Control of Communicable Diseases and Environmental Health. From 1988 to 1995 as Director Bureau of Planning Ministry of Health. In 1995 he was promoted as Director General Community Health and Served this post till 1998. From 1998 to 1999 he served as Senior Advisor to Minister of Health in Community Participation and Health Care Financing. After an early retirement in 1999, he immediately joined WHO-World Health Organization-South East Asia Regional Office in New Delhi starting as Director Health Systems and Community Health and in 2001 as Director Communicable Disease. From March-October 2004 he was assigned as Acting WHO Representative to Thailand. In 2005 he retired from WHO, but in 2006 he was asked to rejoin as Temporary International Professional on Communicable Diseases, at WHO Country Office Indonesia. In 2008 he was reassigned to WHO South-East Asia Regional Office as Director Health System Development till 2009. In 2009 he served as Advisor to the Regional Director until October 2013.
